Staff Nurse LPN (FT)

The Staff Nurse LPN is responsible for the nursing functions of the Senior Services programs. Through the direction of the supervisor this nursing position will provide skilled nursing services in the home and clinic settings for senior services programs.

Typical Hours of Employment: Monday through Friday. 8:00 a.m. to 5:00 p.m.

Essential Duties & Responsibilities
Assures excellent customer service is provided to all participants.
Completes nursing responsibilities including but not limited to monthly medication reconciliation, quarterly medication review with participants, and timely completion of required Functional Capacity Screens and Health Services Plans as designated by regulation.
Promotes and maintains the health of participants through evaluation of care needs, and provides teaching and instruction to meet care goals.
Provides skilled nursing care, by following identified protocols, infection control standards and prescribed treatments.
In conjunction with RN, identifies priorities on a daily basis and provides needed confirmation of critical activities before leaving the site.
Assists in the evaluation of medication compliance and the effectiveness of prescribed medications.
Assists in the transcription of medication orders and communication with pharmacy providers.
Attends and participates in daily meetings, and scheduled participant care meetings including IDT as requested.
Confers with the Interdisciplinary Team for Plan of Care adjustments and enters changes in Electronic Medical Record as agreed upon by the team.
Maintains confidentiality of all participant events, discussions and records.
Demonstrates compliance with organizational mission, vision and values.
Documents provision of care and services in the medical record timely.
Participates in departmental and agency quality improvement activities.
Follows established procedures, protocols, and standard precautions related to nursing services provided.
Properly cleans durable medical equipment as necessary.
Provides for continuity of services in home, community, or clinic through effective information management, both written and verbal communication.
Maintains clinical competency and performs duties within scope of practice.
Promotes interdisciplinary collaboration by prioritizing issues and directing communication to the appropriate team members.
Participates in survey processes for Kansas Department on Aging and Disability Services (KDADS).
Ensures compliance with regulations, professional licensing, and standards of practice.
Demonstrates a working knowledge of providing services to a geriatric population.
Confers with team members for effective and timely resolution of participant needs.
Orients new staff to daily routines, policy and procedure.
Maintains flexibility in schedule and responds to unexpected emergencies and changes in daily assignment in order to meet organizational needs.
Serves in the on-call nurse rotation for all Midland programs including regular, admission and death visits to assure 24 hour care delivery.
Establishes therapeutic relationships with patients and families from a variety of cultural, ethnic, and socioeconomic backgrounds.
Demonstrates interpersonal skills with all levels of medical, nursing, administrative, and other interdisciplinary team members to effectively and efficiently provide level of service required by participants.
Facilitates communication with community agencies including pharmacies, hospital departments, and extended care facilities to optimize participant transitions and the provision of required care services.
Represents organization professionally and promotes positive name recognition in all community activities.
Participates in design or re-design of facilities and systems to promote expansions and service growth.
Accepts census fluctuations and adapts productivity to support organizational goals.
Completes annual educational requirements.
Utilizes evolving technology as instructed.

Current unrestricted State of Kansas License to practice as a Licensed Practical Nurse; Valid Kansas Drivers License. At least one year of experience as an LPN, preferably in home or community health; Must be able to relate well with seniors and their families; Experience working in an interdisciplinary team setting is desired; Computer literate. Must possess valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy.
Physical Demands
Continuously (75 - 100% of time) performs a variety of tasks that involve standing, walking, sitting, stooping, and kneeling. Must be able to stand and walk continuously (75 - 100% of time). Must be able to stand for up to 7 hours at a time to accommodate needs necessary to facilitate clients and program schedule. May have to lift up to fifty pounds maximum less than 25% of the time; May be required to push materials, equipment or furniture less than 10% of the time. The capacity to drive and the availability of a car is preferred.

? Clerical/Clinical: Access to medical records to modify or create. No access to billing or software maintenance.
Exposure Control Category
Category I: This position has the inherent potential for mucous membrane or skin contact with body substance, or a potential for spills or splashes of them. Employees in these positions must follow the Preventative Measures as established by the Midland Care Exposure Control Plan.
Equipment Used
Telephone, laptop computer, FAX machine, motor vehicle, medical equipment as identified for completion of assigned interventions, and other equipment as needed.
Work Environment
Individual must wear appropriate attire all of the time and spends most time inside. Will need to travel to participant homes on occasion where conditions may be adverse due to weather, environment, or participant lifestyle.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
